I understand that I am volunteering my pet to donate blood today. The risks and nature of the procedure have been explained to me. I hereby authorize and direct the veterinarians and their staff to provide such additional services for my pet, which may include administration of sedation as needed in order to allow for safe and stress-free blood donation.

Although the risk involved in donating blood is considered minimal, I understand that my pet may experience mild redness, bruising, or skin irritation at the collection site or have an adverse reaction to sedation. In the absence of negligence, I hereby hold Ethos Veterinary Health and Premier - Grayslake harmless from any and all liability with the respect to this procedure.

I understand that I will receive $25 compensation, or $50 in an emergency situation, for this donation. Compensation is provided in the form of an account credit at Premier-Grayslake or an Amazon gift card. I have selected my preferred method of compensation above.
